Consent - Med Assistance, Administration & Nurse Delegation
This is an informed consent that can be included in your AFH admission agreement. This consent defines the difference of "self-administer"(residents takes own medication) vs. "medication assistance" (caregiver assists to prepare, but resident is able to get the medication where it needs to go- mouth, skin, eye, etc) vs. "medication admistration" (resident requires a caregiver to do the last step/get the medication where it needs to go) and thus requiring Nurse Delegation.
This informed consent can be made part of your admission agreement, signed by the resident or POA, and kept in your records.
